+2 relates to his pulse in his foot. Means that when it was written down he had a pulse there (but have to keep checking to be sure blood supply to the foot remains.)

Oh, he was on everything at that point… and this is a true photo with no photoshopping and nothing is exaggerated. It was a loooonnng healing process but he is a tough guy with a great attitude.
I saw him this morning, and he is still feeling it but is much better.

This is scary because this happened to me( only with a small stick the size of a pencil). the only way you could tell was from the large bump on the opposite side of the wound.
